1. Information Collected.
1.1 Registration. When registering to use the Services, you will be required to provide personal information, such as your name, email address, phone number, and if applicable, payment information.
1.2 Purchases. When purchasing any offerings available through the Services, you may be required to provide information related to your financial qualifications and billing information, including your billing name, address and credit card number.
1.4 All the information we collect, including the above information we collect from you and other users, will collectively be referred to as “User Data”.
- Information Use. User Data is used to provide the Services, and to provide account access, service notifications, and for other purposes related to use of the Services and related offerings. For purchases, we review the financial qualifications of organizations and use your or your company’s data to collect and make payments related to the Services. We may also use User Data for marketing purposes, to operate and improve our Services, and to create new offerings. Information about your use of the Services may be used to provide personalized information about available Services and other offerings from My Urology Doc and our partners.
- Web Beacons and IP Addresses. We use web beacons alone or in conjunction with cookies to compile information about your usage of the Services and interaction with emails from My Urology Doc. Web beacons are electronic transmissions that can recognize certain types of information on your computer, such as cookies, or when you viewed a particular web page. We use the information from web beacons to operate and improve the Services and our email communications, and to provide you with information about My Urology Doc and our Services or and our partner’s services. We collect Internet Protocol (“IP”) addresses to track and aggregate non-personal information. For example, My Urology Doc uses IP addresses to monitor the geographic regions from which users navigate the Site.
- Other uses of User Data.
7.1 Service Providers. My Urology Doc works with other companies that help us run our business. These companies, or service providers, provide services such as delivering customer support, processing credit card payments, and sending emails on our behalf. Some service providers will have access to your personal information in order to provide services to you on our behalf, but we do not permit them to use your information for their own purposes. In addition, we may partner with other companies to jointly offer products or services. If you purchase or express interest in such a jointly offered product or service, we may share User Data collected in connection with your purchase or expression of interest with those partners. We do not control our business partners’ use of User Data, and their use of such information will be in accordance with their own privacy policies. If you do not wish for your information to be shared in this manner, you should not purchase or inquire about such jointly offered products or services. Service providers who manage credit card processing will store, retain and use billing information for the purpose of credit card processing on our behalf.
7.2 Required by Law. My Urology Doc may use or disclose User Data if required by law or if My Urology Doc in good faith believes that use or disclosure is necessary to protect My Urology Doc’s rights, or to comply with a judicial proceeding, court order or other legal process.
- International Data Collection and Transfer. The Services are hosted in and intended for use in the United States. If you are accessing the Services from another country you are responsible for ensuring your use is complaint with any applicable laws, rules and regulations of both the United States and any other country from which you are accessing the Services, and shall defend and indemnify My Urology Doc for and claims arising from your use of the Services from such non-U.S. location.
